The Center for Liver Diseases of the Clinic “AGIOS LUCAS” is a comprehensive care center that focuses on the management and treatment of liver diseases.

How do we treat Scaphoid & Avian Thorax?
Scaphoid thorax

The scaphoid thorax is the condition in which the chest goes inward (pectus excavatum). It affects the position of the ribs and presses on the vital organs. It occurs in one in 400 births and more often in boys. In Greece, more than 20,000 people have this condition. Symptoms of the condition depend on its severity. If it is severe, then the organs of the thoracic cavity are squeezed, affecting vital functions of the organism, such as breathing. The posture and general appearance of the patient are also affected, which can burden him psychologically.

What is the treatment of scaphoid thorax?

Treatment depends on the severity of the condition, age and growth rate of the child. Treatment can be conservative, invasive or mixed.

Conservative treatment is selected when the child is in development and the chest is elastic. The first step is for the thoracic surgeon to indicate to the patient special strengthening exercises and correct his posture. It is also important to constantly monitor the condition so that the doctor intervenes whenever necessary.

Next treatment, considered a combination of conservative and invasive intervention, is the placement of Klobe's cage. It is placed in front of the chest for a few hours a day and acts like a suction cup, pulling the sternum outward. Thus, it does not let the sternum get worse. This treatment lasts up to 18 months.

If the problem is very serious or there is no compliance of the child with the treatments or the child's growth has stopped, then the invasive solution is chosen. This can be minimally invasive or involve classical surgery.

A minimally invasive solution is the installation of the NUSS bar which is placed with a small incision and with the help of a camera under the sternum in order to restore it to its normal position. This bar remains on the chest for 3 years.

The classic surgery is the Ravitz technique, in which we cut and reposition the sternum and ribs. However, it is a laborious and complex operation and is not preferred nowadays. Also the result is often not aesthetically acceptable by the patient.

 

Finally, if the patient does not prefer any of the above operations, they may choose to have medical silicone fitted to fill the gap created by the condition.

Avian thorax

Avian thorax (Pectus carinatum) is a condition in which the thorax and ribs protrude outward and occurs in one per 100,000 births. The main symptoms of the condition concern the posture but also the general appearance of the patient, which can burden him psychologically. If the condition is severe, affecting vital functions of the body such as breathing.

What is the treatment of avian thrush?

Treatment of avian thorax is either conservative or invasive.

Conservative treatment is the first option and is done when the child is in development and the chest is elastic. It includes:

  • special belts that press the chest inward; They are properly regulated by the doctor so that they are acceptable to the child and do not cause a feeling of pressure.
  • Suggestions from the doctor for posture during the day and sleep

When the problem is severe and cannot be treated conservatively or the patient's development has been completed, then we proceed to invasive options. They are divided into classical surgical and minimally invasive.

Classical surgical treatment:

It is the reverse Ravitch, in which we cut and reposition the sternum and ribs. However, it is a laborious and complex operation and is not preferred nowadays. Also the result is often not aesthetically acceptable by the patient.

Minimally invasive treatment:

  • The most common option is to place a special bar outside the sternum, which presses the chest inward.

  • Another option is to enter the chest with a camera and rub the problematic sides with a special tool to reduce the forces that push the sternum outwards. Next, we insert a special belt, which can now work and effectively press the sternum inward. The thoracic surgery team of the clinic “Saint Luke” is one of the few in the world to apply this technique

  • A third option, which is preferred by women at an older age, is breast prosthetics. In this way, the problem is covered and aesthetically corrected.

Therefore, the doctor can intervene conservatively in the condition, without burdening the child with some heavy intervention. Therefore, the treatment of congenital diseases must begin with childhood. If, after the completion of growth, conservative methods have not brought the desired result, surgical intervention becomes necessary, because now the chest ceases to be elastic.
